Project REACH

Project REACH offers permanent individual housing for homeless men, women, and families with disabilities. Regular case management sessions are provided to monitor each participant’s progress toward independence.

CHARTING A PATH TO INDEPENDENCE

During sessions, the Case Manager and the participant review the participant’s adherence to:

  • Budgeting and managing expenses
  • Time management
  • Conflict resolution
  • Employment goals
  • Goals for obtaining education, addiction counseling and/or treatment for mental and physical health

Participants may also use services offered at Our Daily Bread Employment Center, including:

  • Job readiness training
  • GED preparation referrals
  • Alcoholics Anonymous meetings
  • Narcotics Anonymous meetings

Eligible participants are persons with

  • Serious mental illness
  • Chronic health issues
  • Alcohol and/or drug addiction
  • HIV or aids related diseases

All referrals for Community Housing programs come through Coordinated Access within Baltimore City. When housing opportunities come available, staff will inform the Baltimore City Coordinated Access team who will alert referring organizations. Once potential participants are screened for eligibility and required documentation is collected, referrals are made to the programs. Potential participants are screened by program staff and once a match is determined, housing placement occurs.
